Recently, electrical equipment used as connection leads for various amusement rides has been found to use single core cables with single pin plug/socket connections (Pictures 1&2). These connections have been used to supply single or multiphase power to electrical equipment in an uncontrolled environment. The Electrical Safety Office (ESO) and Workplace Health and Safety Queensland (WHSQ) are investigating the fatality of a ceiling insulation installer which occurred on 18 November 2009 at Stanwell, near Rockhampton. A second person assisting with the work received an electric shock.
An investigation is currently underway. Initial findings suggest that a fault in the electrical installation in the ceiling cavity is the likely cause of the fatality. A cable supplying the stove circuit was damaged by a ceiling fixing screw causing the supporting metal ceiling batten to become energised at approximately 240 volts. This is the second electrocution death of an insulation installer in the last four weeks. Read more

Prior to Christmas, a person was killed on a property in rural Queensland adjacent to a high voltage transformer. At the incident site a kite was found to be tangled up on a 19,000 volt powerline near the deceased. It is suspected that an aluminium pole on the ground was used in an attempt to free the kite from the powerline. Read more
