Employers

Receiving a Fine Collection Notice for the redirection of earnings from SPER means that you, as the employer, are required to deduct the weekly amount as noted on the Fine Collection Notice, and pay that to the Registrar of SPER.

Should the employee have any queries or concerns regarding their SPER matters or the issue of this Fine Collection Notice, advise them to contact SPER.

The following Fine Collection Notice forms are to be used when sending information to SPER:

Cessation of Employment SPER3005 (PDF File, 29.3 KB) – This form should be completed if the debtor has never been an employee or ceases employment with your organisation.

Return of Payments SPER3011 (PDF File, 66.9 KB) – This form should be completed and sent with any money being remitted to SPER.

Return of Non-payment SPER3010 (PDF File, 41.9 KB) – This form should be completed when money cannot be remitted to SPER for a reason other than cessation of employment.
