Complaint form for children and young people
If you want to make a complaint, we will listen, treat you with kindness and let you know how long it might take to see an outcome.
For help filling out this form you can ask someone you trust for support, like:
- your family
- a friend
- one of our staff members
- a community Elder.
If you are 18 or over, you may like to use our adult online complaints form instead.

Privacy statement
What information we are collecting
We (the Department of Justice or DoJ) are collecting the information that you choose to give us, including:
- your name
- your contact details
- sensitive information, such as your health information
- any other information you choose to provide to us.
You may choose to give us your email address. It may be supported by a server outside of Australia. For example, the United States of America. If it is, the personal information we send to your email address will be sent to the server outside of Australia.
Laws that let us collect your information
Section 11 of the Child Safe Organisations Act 2024 allows us to collect your personal information for these purposes.
How we use your information
We are collecting your information to identify and deal with your complaint according to our Child safety and wellbeing policy.
We may collect personal information about you from another person or entity to investigate and respond to your complaint.
We may give the personal information we are collecting to any of these agencies, where required:
- Queensland Police Service
- Queensland Family and Child Commission
- Crime and Corruption Commission
- Department of Families, Seniors, Disability Services and Child Safety.
We will not use your personal information for any other purpose unless we are authorised or required to:
- under an Australian law
- by a court or tribunal order.

- how to make a complaint about the way we have handled your personal information
- how your complaint will be handled.
